   I have read the Terms and Conditions for Agent Onboarding, which describes the use of Electronic Records, Disclosures, Notices, and e-mail. I consent to the use of Electronic Records in connection with my application for agent contracting and / or agent appointment with Guarantee Trust Life Insurance Company (“GTL”). I have been able to view this information using a personal computer or other device, which is capable of accessing the Internet. I have an account with an Internet service provider and I am able to send e-mail and receive e-mail with hyperlinks to websites. I am also able to download and review files in Portable Document Format (“PDF”).
 
   By using GTL’s Agent Onboarding I also consent to the use of Electronic Signatures in connection with my application for general agent contracting and / or agent appointment with GTL in place of written documents and handwritten signatures. I understand that I will be bound by these Terms and Conditions.

   Please read these Terms and Conditions for Electronic Signature, Disclosure and Consent carefully. By consenting to apply for General Agent and / or Agent Appointment using Guarantee Trust Life Insurance Company’s (“GTL”) electronic Agent Onboarding process, you agree to be bound by these terms and conditions.
 
  Electronic Delivery of Required Documents, Disclosures and Notices
   You consent to review and receive all required documents, disclosures, notices, terms and conditions, and other documents and all changes to the required documents, disclosure notices, etc. electronically.
 
  System Requirements to Access Information
      A personal computer or other device, which is capable of accessing the Internet. Your access to this page verifies that your system / device meets these requirements.
      An Internet web browser which is capable of supporting 128-bit SSL encrypted communications, which requires a minimum web browser version of Microsoft® Internet Explorer version 11.0 (available for downloading at https://support.microsoft.com/en-us/help/17621/internet-explorer-downloads),
Microsoft Edge ,
Google Chrome (available for downloading at https://www.google.com/chrome/ ),
